Effective, February 19, 2007, the Civil Union Law will go in to effect.  Please note, as has been the practice in the past, appointments must be made for all applications for:  Marriage, Domestic Partnership and Civil Unions.  You may call the Deputy Registrar’s Office at: (609) 395-0900, ext. 231 to schedule an appointment between the hours of 8:30 a.m. – 4:30 p.m., Monday through Friday.

Forms for Marriage, Domestic Partnership and Civil Unions may be obtained by accessing the State Office of Vital Statistics web site at: www.state.nj.us/health/vital/forms.shtml.
